Which of the following most accurately describes the problem?

by Phil Cram • The inquest has opened on a Cumbrian truck driver who was killed by a falling load just weeks before he was due to retire.

Bobby Bell, 64, of Maryport, died instantly when a part of his load of fabrications slipped and crushed him while unloading at British Steel's track products division in Workington, Cumbria.

Bell, who was due to retire on 7 January, had worked for lreby-based haulier AD Boyes for 10 years. He was regarded as one of their best drivers.

The accident happened on 5 December on a roadway in the middle of the plant close to the doors of the roll shop.

Emergency services rushed to the incident but were unable to do anything for Bell, who was with a co-driver at the time of the accident.

Health & Safety Executive officials were quickly on the scene and cordoned off the area in preparation for a full investigation, which started the following day.

Tony Williams, managing director of British Steel's Workington plant, declined to speculate on the causes of the accident.

Robbie Boyes of Al) Boyes says: "Bobby was not just a driver, he was also a friend and we are all very upset. He never had time off and was very reliable—if you wanted a job doing, Bobby was the man."

The inquest was adjourned until next month.
