Which of the following most accurately describes the problem?

• A London-based company and its directors have been banned from holding an 0licence for three years after showing a "complete disregard for tachograph regulations".

South-East IC Michael Turner told a public inquiry that Dev, of Hackney Wick, had been under investigation for hours and maintenance problems since November 1996.

The first investigation found 36 offences on 80 tacho charts with more than 18,000km unaccounted for. A public inquiry was called for May 1997 but no disciplinary action had been taken as steps were taken by Dev to improve its operations.

However, after the inquiry the three vehicles presented by Del, for annual tests showed a "woefully inadequate repair and inspection process".

Inspectors found 29 assorted defects on the three trucks including some which Turner said were "easy to spot", adding: "I am amazed a company could produce vehicles in this condition to an annual test." Another inspection of 79 tachograph charts uncovered 41 offences with more than 500km missing.

Turner revoked Dev's licence, saying the company had made "little effort to comply with the law". He gave transport manager John Butler 21 days to tell him why he should be considered to be of good repute.
