ncident: A mechanic was burned when using an o'-acetylene torch to cut brackets holding the exterior pipework of a road tanker used for carrying crude oil, which caught fire. The case was brought because of a failure to carry out a risk assessment or to provide a safe system of work when working in the confined space inside the tanker and on the pipework on the exterior of the tanker.
