Inquest shows workmen were killed by equipment and tools flying forward following a van collision. Collin Barnett reports.

AN INQUEST HAS revealed that two workmen were crushed to death in a Volkswagen Transporter van which braked hard, sending equipment and tools flying forwards.

The two other occupants of the van were seriously injured in the road traffic accident at Penybont station on the A44 near Llandrindod Wells.

The Welshpool coroner heard how the four men were travelling in the three-seater Volkswagen from their homes in the Kidderminster area to a job at Rhayader.

One of them was riding in the load space with an assortment of tools and equipment when the van collided on an S-bend with an oncoming Calor Gas tanker.

Police investigators decided hard braking had caused the van's wheels to lock so the van understeered into the opposite lane.

The investigating pathologist reported that John Burden, 19, and Paul Ingram, 27, were crushed from behind by a heavy object or objects caused by rapid deceleration. The coroner recorded verdicts of accidental death resulting from a road traffic collision. The truck driver escaped with minor injuries.

CM has long advocated the fitting of suitable bulkheads. The limitations of current legislation mean the Health & Safety Executive was unaware of this incident; there is no requirement for road accidents at work to be reported. Recently, however, there have been moves towards creating a closer relationship between the police and the HSE in road accident investigations (CM 14 July).

A review is being conducted into the RIDDOR dangerous occurrence reporting system; one of the proposals is to bring such accidents within its scope.

The public consultation period for the RIDDOR proposals has now ended and comments are being reviewed.
