Which of the following most accurately describes the problem?

• A misadventure verdict was recorded on a coach driver who received injuries which led to his death, when a coach rolled back in a repair bay at the St Helens Industrial Co-op's coach depot, Clipsley Lane, Haydock. The driver, John Brady, 54, of Sutton Heath, St Helens, died from broncho-pneumonia brought on by chest injuries.

Mr Cohn Dyson, assistant manager of the depot, said Mr Brady's coach had been parked broadside to another coach in the bay. The driver of the other coach, Anthony Witham, said his vehicle had been in the repair bay for a brake adjustment. He sat pumping the pedal while a mechanic made adjustments. The handbrake was off. Later, he started the engine and was waiting for the other coach to move when he was called away. When Mr Witham returned with the mechanic the coach had rolled back. "I assume the handbrake was still off, although it isn't like me at all," he added.
