Which of the following most accurately describes the problem?

THE HIGHWAYS Agency (HA) has explained why there were no fog warning signs prior to the 34vehicle M5 crash on 4 November.

As the inquiry into the deaths of seven people is launched, an HA spokesperson says: “At the time of the incident our Regional Control Centre at Avonmouth had not received any warnings of adverse weather conditions.” The HA said a weather warning had been issued in the morning predicting there could be slight and isolated fog, but this warning was for the whole of the SouthWest; an update followed later which conirmed no hazards were predicted.
