Which of the following most accurately describes the problem?

A DONCASTER operator escaped with a warning about its maintenance standards after replacing its fleet with new vehicles. It blamed poorrecord-keeping,on the theft of its 'office'. Pellantry Services appeared before North-Eastem Traffic Conunissioner Tom Macartney at a Leeds disciplinary inquiry. The company holds a licence for four vehicles and four trailers.

Vehicle examiner Tony Aitken said a maintenance investigation was prompted after an artic belonging to the firm crossed the hard shoulder of the M'18 and rolled down the embankment. Few records were produced and none existed before July 2006 due to the theft of a portable building, which was the office. Records were available for only one vehicle, as those for the other had not been sent by the maintenance contractor. Records for newly acquired replacement vehicles, being maintained by the dealership, were satisfactory.

Director John McGuinness said that at one time the firm had operated up to 30 vehicles on site. It had initially changed maintenance contractors because it was not satisfied with the service. It was during the three-month period when the second contractor was being used that most of the prohibitions arose.

Producing the crime report number,John Reid, for the firm, said the theft was in September 2005. He did not know what had happened to subsequent records.

McGuinness said he did not deny vehicle maintenance had been unsatisfactory. The problem had been running the two sides of the business. He had now disposed of the site business and those vehicles had gone four new Renaults had been purchased. Also, a replacement transport manager had been recruited; the previous one had been ill for 18 months.
