Which of the following most accurately describes the problem?

In addition, the company's director has been disqualified from holding an 0-licence due to loss of repute. Chris Tindall reports.

A PLANT TRANSPORT firm has had its licence revoked and its director has been disqualified from applying for a licence after it falsified documents and failed to retain vital maintenance records.

A Cambridge public inquiry heard how a Vosa investigation into Moore Plant Hire revealed evidence of false addresses and phone numbers on 0-licence application forms.

Gary Moore and his wife Maria Moore, respectively director and company secretary for the Bucks-based company, failed to appear before Deputy Traffic Commissioner Mary Kane. The firm, which had a standard national licence for one vehicle and one trailer, had been operating since May 2005. Suspicions were aroused when Vosa realised the address used for the nominated transport manager, Daniel O'Connor, was connected to someone who had a number of aliases and had lost his repute several years ago. The phone number given on the form was unattainable.

In addition, O'Connor had apparently spelt his name incorrectly on an 0-licence form.

An inspection of the company's drivers' records revealed missing mileage and a failure to retain vital documents. Vosa found no evidence of systems in place to record vehicle defects and O'Connor had not exercised the required "continuous and effective" responsibility.

Traffic examiner Bernadette Williamson reported that Gary Moore had failed to comply with the conditions and undertakings stated on the 0-licence.

Kane concluded: am satisfied that there are breaches, and am revoking the firm's licence. I am also satisfied that Gary Moore's repute has suffered to the effect he should be indefinitely disqualified from applying for an 0-licence."
