Which of the following most accurately describes the problem?

• A multiple pile-up on the M6 near Wednesbury on Wednesday 7 January left two truck drivers dead.

The incident began at 06:15hrs when a southbound lorry driven by Joseph Malloy, 53, of Warrington, ploughed across the central reservation and collided with two lorries and two cars.

Tony Bufton, 50, who was heading north, was killed in the collision, as was a car driver. Malloy died later from multiple injuries.

Police are denying reports that Malloy was thrown clear from his cab, suggesting he was not wearing a seatbelt at the time of the smash. The incident is still being investigated.

No names of companies involved have been released by police, but one of the trucks is known to belong to Halls Roadway of Alcester, Warwickshire.
