Which of the following most accurately describes the problem?

A WELSH HAULIER has been fined £6,000 with £5,000 costs after a truck driver was run over by a forklift in its yard.

Ammanford operator TD Williams (Brynamman) pleaded guilty to failing to ensure the safety of its employees.

Driver James Martin Spain Anderson. of Llanelli, injured his ankle in the accident. which happened when he was hitching his tractor unit to a trailer in March last year. After the hearing at Ammanford Magistrate's Court, Health and Safety Executive (HSE) inspector Phil Nicolle said: "The case demonstrates the dangers of vehicles working in the vicinityof pedestrians. Fortunately. the injuries sustained by Mr Anderson were not life-threatening, but so many cases have ended in tragedy.

"This is a serious workplace transport incident involving a company whose core business is transport. The company admitted having no suitable or sufficient risk assessments in place for the loading bays, where this incident occurred, despite previous advice from HSE and there was inadequate segregation of vehicles and pedestrians."

About 70 people are killed and 2.000 injured every year in workplace accidents involving vehicles.
