Which of the following most accurately describes the problem?

• A truck driver and three other motorists were killed in a six-vehicle pile up on the M6 in Cheshire last week.

The incident happened last Wednesday (5 September) at around 11:40hrs between Keefe Services and Junction 16.

A truck travelling southbound is believed to have suffered a tyre blowout, causing it to smash through the central

reservation. A van following the truck ended up on its side on the northbound carriageway. The drivers of both vehicles were pronounced dead at the scene.

Two cars travelling on the northbound carriageway crashed into the wreckage, killing both drivers instantly. Two other car drivers escaped serious injury when they swerved to avoid the accident.
