Which of the following most accurately describes the problem?

• When Mr G. Sharp, a DoE vehicle examiner, visited the premises of A. E. Jones (Removals) Ltd, Birmingham, he found them to be a suburban house with practically no vehicle maintenance facilities whatsoever; a vehicle outside on the road had a wheel missing.

This was the story heard by Mr John Else, the West Midland LA, when, in Birmingham last week, he curtailed the company's 0 licence by removing the two-vehicle margin and suspending the one vehicle in possession for two months.

The vehicle examiner had told the LA that when inspected the vehicle on the roadside was issued with an immediate GV9; no records had been kept relating to that or another vehicle which the company operated.
