Which of the following most accurately describes the problem?

• A coroner has warned drivers directing L(iVs reversing into loading bays that they should stand at least two feet back from the edge of the bay.

An accident which killed a lorry driver in May could have avoided if this precaution had been followed, said Paul Rose, a coroner sitting at Croydon, Surrey last week.

The inquest heard that John Adair, 45, of Hill Rise, Darneth, Kent was crushed while directing another lorry reversing into a loading bay at Tip Top Bakeries in Cray Avenue, Orpington on 27 May.

Andrew Dodwell, of Compton Place, Erith, Kent told the coroner that Adair offered to see

him back into the bay: "I reversed. John was on the platform waving me back," he said. "The angle was wrong and he indicated me to stop. I got out and he told me I would have to move the lorry to get it parked straight. I moved it and reversed slowly."

Once his vehicle got within three or four feet of the bay, he was not able to see Adair standing there. Dodwell said he could have easily have driven the lorry back without help but Adair was doing it as a favour. "It was only when I got out I realised what had occurred."

On the direction of the coroner, a jury returned a verdict of accidental death.
