AN INVESTIGATION is under way aftera truck driver was crushed to death when a forklift truck he was delivering fell on him.

The incident happened at 4pm on Monday 13 September as Michael Powers was unloading the forklift at engineering firm Windsor Komatsu on the Armstrong Industrial Estate, Washington, Tyne & Wear.

A Health and Safety Executive spokesman says: "Staff from nearby company Life Engineering tried to rescue the driver by using their own forklift in an attempt to lift the vehicle off him. An investigation into the incident is being carried out."
