Which of the following most accurately describes the problem?

KANE HAULAGE has been fined £11,700 after it carried out demolition work on buildings while electricity supplies remained live.

In a Health and Safety Executive (HSE) prosecution, City of London Magistrates’ Court was told how the St Albans, Hertfordshire-based haulier had been contracted to carry out demolition work at the Oldfield Trading Estate, Sutton Coldfield, in 2009.

Utility company EDF Energy had warned Kane not to continue with work at the site until all the electricity supplies had been disconnected. However, despite the warning, Kane carried out work while electricity to the buildings undergoing demolition remained live.

Up to 20 severed live gas pipes were later discovered at the site, and an estimated half a tonne of gas had been released.

Kane – authorised for 50 vehicles and 10 trailers – pleaded guilty to breaching Section 3(1) of the Health and Safety at Work Act 1974. It was also ordered to pay costs of £6,936.50.
