Which of the following most accurately describes the problem?

• A truck driver was facing disciplinary action this week after his skip carrier ran down a hill for more than a third of a mile before flattening a parked car.

The 32-tonne Foden eight-wheeler is owned by South Herts Waste Management, which is contracted to operate Nottingham City Council's civic amenity site in Colwick.

The unladen truck was parked outside the site when it rolled away round a steep bend, flattened a lamp post, narrowly missed a group of workers having lunch and finally smashed into a Nissan Prairie car.

South Herts technical manager Ian White says the company is still investigating.
