Which of the following most accurately describes the problem?

A WEST MIDLANDS company that failed to appear at a public inquiry in order to face questions about its safety record has had its licence revoked.

West Bromwich-based Polytech Plastics, with a two-vehicle licence, had been called before Lester Madrell. the West Midlands Deputy Traffic Commissioner. When no-one turned up, the DTC revoked the firm's licence and disqualified its sole director Jagtar Singh from holding an 0-licence indefinitely.

The DTC said that last August, the company was convicted of permitting a driver to drive when not properly licensed — the driver concerned only held a Belgian driving licence.

Also, an 'S' marked prohibition was issued in October for a worn tyre. When the vehicle was produced forprohibiti on clearance, a variation notice was issued for a series of other defects, including steering and suspension faults, a loose body panel and a fuel leak.

Vehicle examiner Nigel Davies said he had carried out an unannounced maintenance check. Only two inspection records could be produced, dated October 2005 and February 2006. There was no written driver defect reporting system, and it was obvious daily checks were not being carried out. There was a 100% initial failure rate at annual test.
